Tuberculosis (TB) remains the most common opportunistic infection affecting people living with HIV in India. Because both conditions interact directly with the immune system, managing TB/HIV co-infection requires careful clinical coordination.
Recognized Symptoms of TB in People Living with HIV.

Unlike classic TB presentations, symptoms in immune-compromised individuals can be subtle:
Persistent fever or unexplained night sweats
Chronic cough lasting more than 2 weeks
Significant, unexpected weight loss and fatigue
Swollen lymph nodes (particularly in the neck)

Principles of dual treatment:

1. Prioritizing TB Treatment: Anti-TB Treatment (ATT) is started immediately upon diagnosis.

2. Timing the ART Start: To prevent Immune Reconstitution Inflammatory Syndrome (IRIS)—a temporary inflammatory flare-up as the immune system recovers—ART is typically introduced 2 to 8 weeks after starting ATT (depending on CD4 count).

3. Managing Drug Interactions: Rifampicin (a core TB drug) interacts with certain ART medications. Our doctors adjust dosages (such as doubling Dolutegravir doses) to maintain viral suppression.
